I offer comprehensive treatment programs designed specifically for you so you can successfully achieve your goals and return to your active lifestyle. From initial consultation and treatment to ongoing training, nutrition, and education, I offer a variety of services to assist you in regaining strength, recovering from injury, and returning to your daily activities.
I also offer a Home Listening program to facilitate healing in your times of busy schedules. Not only do you have the equipment for your own use, I’m available via Skype, phone or email to help you or your child navigate the program with ease.

By having you complete these forms prior to your scheduled appointment, I can spend more time during the visit working with you. For new patients, please complete these forms prior to your first visit and bring them with you on the day of your scheduled appointment.
I will retain these completed and signed forms in our office files. If you are unable to complete these forms ahead of time, please arrive approximately 15-20 minutes prior to your scheduled appointment time to complete them prior to your visit.

I generally start by scheduling six, 50-minute appointments. A treatment strategy and plan of care is established after your initial evaluation and assessment. I also offer one-time consultation sessions. It is important to note that your medical insurance is a contract between you and your insurance carrier. It is your responsibility to understand your coverage and what they will pay for physical therapy. Any balance not paid by your insurance is yours to pay. You will receive a statement once your insurance has responded to my claim. Providing me with accurate insurance information frees me to concentrate on you and your health.
If you weren’t able to complete your appointment paperwork prior to your initial visit, the forms will be provided to you. Also, copies of your insurance card(s) and photo ID will be made at this time. All appointments start on the hour and last 50 minutes, unless otherwise noted. Please arrive 15-20 minutes early for the first visit to complete the necessary paperwork. Arriving on time allows you to take advantage of your full treatment time. During the first visit, I will conduct an evaluation of your injury and collect data to establish a custom treatment plan and physical therapy goals based on your individual needs. I appreciate your honest communication. There is an answering machine available at all times and your call will be returned as soon as possible. Email is another good form of communication and I will respond within 24-hours, unless otherwise noted.
Please make sure that you bring your insurance information/card with you. You will be expected to pay your co-pay and any deductible that is required at the time of your visit. You may pay with Visa, MasterCard , HSA or a check. You’ll need to bring or wear comfortable clothing.
Partners in the Healing Process
I will conduct a one-hour comprehensive evaluation by taking a complete medical history followed by a detailed physical examination. After the examination, I will educate you on the findings and explain the diagnosis then I will outline the physical therapy treatment plan. This will involve an assessment of your whole body, looking at posture, range of motion, balance and strength levels. I’m also very interested about all aspects of your life that may contribute to the picture of who you are. Each patient is treated uniquely, and I listen closely to the information that you and your physician provide. I’ll take the time to get to know you, your health history and your individual concerns and goals. This allows me to perform a complete evaluation. I view you as a whole person, not as an injured part. This is the basis of my holistic physical-therapy philosophy.

There is plenty of on street parking. The easiest way to get to the office is to come in the East side door- it faces the apartments on 8th street. The office will be the first one you come to when exiting the first floor stairwell. #150. Come down the hall and you will see the waiting room.
